To submit a warranty claim, please complete the required fields below. You must provide an accurate description of the claim area that you are making. You must also provide accurate photos of the problem. Be detailed in your description and provide as many photos as possible.

This form must be filled out completely and submitted with photos of each claim area, description(s) of the claim areas before the claim will be evaluated for approval.
Please allow 2-4 weeks for all claims to be evaluated. You may be contacted by a representative during this time for additional information.
Product/material replacements will be made upon product availability. Product may not be available at the time of claim processing. If not available, a replacement order will be back-ordered and then repair scheduled upon availability.
If the exact product/material cannot be procured, a like kind replacement will be used.
Substitutions of products, colors, styles, sizes, etc. Will not be accepted.
